Custom Order Request
★Hello! Welcome to Mad Cutter Designs! ★
★ We create unique and personalized home decor, apparel, gifts and other cool stuff! Custom handcrafted items, including heat pressed apparel, custom signs, decals, engraving and other gifts.
★ We are happy to work with you to create the perfect item for yourself or as a gift to another or your business.
★ Current turn around time is 2-3 weeks from time of payment. Excluding weekends and holidays. (Will update as it changes). I will contact you as soon as I get to your order form on my list.
★ This order request form MUST be filled out to be valid. We do have the right to decline orders.
★ Payment is due upfront. Since all items are created custom, cancellations and returns are not available.

★ Deposits can be made on orders $200+. All other orders must be paid in full.
★ Discounts are available for bulk orders of 12 or more of the same item.
★ If you wish to resale Mad Cutter Designs items. You must let us know and provide a resale license.
★ Orders will NOT be worked on until payment is made.
★ If personalization isn't specific at time of order this may cause a delay in your order.
★ If you need the order done by a certain date PLEASE let me know at time of order. Depending on the time frame there may be an additional $15 rush fee. Or you may to choose to pay the $15 rush fee to push your order to the top of my list. If you would like this, let me know... otherwise I will let you know if this applies to you or not.
★ If personalization isn't specific at time of order this may cause a delay in your order.
★ Designs are NOT to be copied for reproduction or for personal use. Mad Cutter Designs owns ALL designs. We personally came up with the design(s) and/ or we have bought the rights to use them. Please do not ask us to other shop designs, as we will decline your request.
★ At this time Mad Cutter Designs does NOT sell any of our designs.
★ There will be a $5 non refundable design deposit if I need to draft a new design from scratch. I will let you know if this applies to you or not.
★ If you need the order done by a certain date PLEASE let me know at time of order. Depending on the time frame there may be an additional $15 rush fee. Or you may to choose to pay the $15 rush fee to push your order to the top of my list. If you would like this, let me know... otherwise I will let you know if this applies to you or not.
★ If you do not pay within 2 weeks, your order request will deleted. You will have to resubmit this form to get back on my order list.
★ Local pick up only available at Copeland's Camp N Ride in Paradise, CA.
★ US only shipping available.
★ Mad Cutter Designs will NOT disclose any personal information when it comes to specific equipment we use, supplies, vendors, etc that we use.
★ We stand by our product 100%. If you should experience any issues with our product please let us know ASAP with pictures and we will work with you to solve the problem.
★ By completing this form you understand and agree to Mad Cutter Designs policies as stated above.
